In order to comply with current legislation on data protection, especially Law 1581 of 2012 (and other rules that modify, add, supplement or develop it) and Decree 1377 of 2013, below we inform you of the relevant aspects in relation to the collection, use and transfer of personal data that Consulens S.A.S. platform (hereinafter the “Platform”), performs on your personal data, under the authorization granted by you to advance such treatment, as well as the handling.
In this policy for the processing of personal data (the “Policy”) you will find the corporate guidelines and law under which the Platform performs the processing of your data, the purpose, your rights as owner, as well as internal and external procedures for the exercise of such rights.
The Platform will be responsible for the treatment and, therefore, may collect, store, use (Include the operations to be performed with respect to personal information collected) for the following purposes: to make available for consultation on request the information that is recorded in the Platform by me when I am registered in the same.
I state that I was informed that in case of collection of my sensitive information (this only if sensitive data is collected), I have the right to answer or not the questions I am asked and to deliver or not the requested data, whether these data are my own or of minors to whom I am representing.
I understand that sensitive data are those that affect the privacy of the Holder or whose improper use can generate discrimination (For example: political orientation, religious or philosophical convictions, data relating to health, sex life and biometric data).
I declare that I have been informed that the sensitive data that will be collected will be used for the following purposes: to make available for consultation on request the information that is registered in the Platform by me when I am registered in it.
HOLDER’S RIGHTS
Your rights as owner of the data are those provided for in the Constitution and Law 1581 of 2012, especially the following:
AUTHORIZATION FOR THE PROCESSING OF PERSONAL DATA
a) Access free of charge to the data provided that have been subject to processing.
b) Request the updating and rectification of your information in the event of partial, inaccurate, incomplete, incomplete, fractioned, misleading data, or data whose processing is prohibited or has not been authorized.
c) Request proof of the authorization granted.
d) File before the Superintendence of Industry and Commerce (SIC) complaints for violations of the provisions of the regulations in force.
e) Revoke the authorization and/or request the deletion of the data, unless there is a legal or contractual duty that makes it imperative to keep the information.
f) Refrain from answering questions about sensitive data or data of children and adolescents.
I may exercise these rights through the channels or means provided by the Platform for public attention: the email admin@consulens.co, for the attention of requirements related to the processing of my personal data and the exercise of the rights mentioned in this authorization.
For all the above, I have given my consent to the Platform to treat my personal information in accordance with the Personal Data Processing Policy provided by the company in electronic media and made known to me before collecting my personal data.
If the account I am trying to create on the Platform corresponds to an account with data of minors I certify that I am the person legally authorized to treat that information and I am responsible at all times for that information.
I certify that I am solely responsible for the information I register in the Platform and that such information is true and accurate and I agree to keep it updated understanding that it may be used to provide me with emergency medical services. I certify that I understand that the information may be published and used to treat me or the minor I represent in a possible emergency case as vital information that I wish to share to help others save my life or the life of the minor I represent.
I declare that this authorization was requested and given to me prior to the release of my information and that I freely and voluntarily sign it once I have read it in its entirety.
